When you request a shirt, Izzi's Gym collects the name and email address or phone number you provide, along with the item, size, quantity, and any note you add.

WHAT WE USE IT FOR

We use this information to organize the weekly supplier order, answer questions about the request, and tell you when the item is ready for pickup. We do not use this form to take payment.

WHO RECEIVES IT

Izzi's Gym staff can access requests. Supplier order sheets use the product and quantity information needed to place the wholesale order; customer contact information is not sent to the suppliers as part of the order.

HOW LONG WE KEEP IT

Request records may be kept for up to one year for pickup support, returns, and store records, then removed when no longer needed.

YOUR CHOICES

To correct or remove a request, bring your reference number to Izzi's Gym at 807 N Lake Ave in Sioux Falls or contact the shop using the information on the main Izzi's Gym website.
